Can pH monitoring reliably detect gastro-oesophageal reflux in preterm infants?

چکیده
Gastro-oesophageal reflux (GOR) is the regurgitation of gastric contents into the oesophagus. It is alleged to be the cause of many clinical problems in premature infants, such as failure to thrive, apnoea, desaturation, bradycardia, and stridor. 2 Twenty four hour oesophageal pH monitoring is currently regarded as the optimal method of diagnosing occult GOR in infants with respiratory events. European and North American working parties have produced guidelines for the methodology and interpretation of oesophageal pH studies in infants and children. 4 However, pH monitoring in preterm infants has shortcomings, which makes the diagnosis of GOR in this group more diYcult and its clinical significance more uncertain than in older children. This review seeks to highlight these shortcomings and to discuss the future of pH monitoring in preterm infants.

Does mechanical ventilation precipitate gastro-oesophageal reflux during enteral feeding?
The influence of intermittent positive pressure ventilation on gastro-oesophageal reflux in preterm infants is not known. In many neonatal units, however, concern that ventilation may increase gastro-oesophageal reflux (and therefore aspiration) leads to avoidance of enteral feeding during ventilation. We have therefore performed a crossover study of gastrooesophageal reflux by monitoring lower...
متن کاملGastric emptying and gastro-oesophageal reflux in preterm infants.
Gastro-oesophageal reflux is common in preterm infants, but the role of gastric emptying as a causal factor has not been studied before. Gastric emptying was therefore measured in 19 healthy preterm infants (median gestational age 32 weeks) while concurrently measuring 24 hour lower oesophageal pH, using an antimony pH electrode, positioned manometrically. Real time ultrasonic images of the gas...
متن کاملGASTRO-OESOPHAGEAL REFLUX Mechanisms of gastro-oesophageal reflux in preterm and term infants with reflux disease
Background: Transient lower oesophageal sphincter relaxation (TLOSR) is the predominant mechanism of gastro-oesophageal reflux (GOR) in healthy infants but the mechanisms of GOR in infants with GOR disease (GORD) are poorly understood. Aims: To measure the occurrence of TLOSR, GOR, and gastric emptying (GE) rate in preterm and term infants with GORD. Patients: Thirty six infants were studied an...
متن کاملMultipoint measurement of intragastric pH in healthy preterm infants.
BACKGROUND The diagnostic use of 24 hour oesophageal pH monitoring in infants is controversial because of the impact of feed buffering of gastric acidity. AIM To re-evaluate the effect of feeding on gastric pH and reflux using multipoint pH measurements. SUBJECTS Fifteen healthy premature infants fed every four hours. METHODS Oesophageal pH and intragastric pH at 3, 6, and 9 cm below the ...
